What FixLoop is and how the loop works

Write down what went wrong, find out why, fix it, and check it stayed fixed.

FixLoop helps a team deal with problems at work so they do not keep coming back. Every problem goes through the same steps, on one screen.

  1. Report it. Say what happened, where, when and how serious it was. Add what was done right away under Make safe / Containment.
  2. Root cause. Find out why it happened, by asking why a few times or by filling a simple cause map.
  3. Fix it. Write one real change, pick who owns it and when it is due.
  4. Effectiveness verification. Later, FixLoop asks: did the problem come back?
  5. Sign off. A named person reads the steps and closes it.

If the problem came back, it goes back to Root cause, never straight to closed. A problem only closes once a person signs it off.

What the statuses mean

  • Root cause: reported, now finding out why.
  • Fixing: the cause is picked and the fix is being done.
  • Verifying: the fix is done, and FixLoop is waiting to check it held.
  • Ready to sign off: waiting for someone who can sign off.
  • Closed: signed off.
