Find the root cause with five whys or a cause map

Ask why until you reach something you can change, or map the possible causes.

Anyone on the team can work on the root cause. Open the problem and go to the Root cause step. Pick one of two ways.

Ask why

  1. Answer the first question: why did this happen?
  2. Press Ask why again and ask why about your answer. Most problems need 3 to 5 whys.
  3. Stop when the answer is a step, tool, layout or habit you can change.

Cause map

  1. Add anything that may have played a part, under People, Process, Place or Tools.
  2. Tap the one or two real causes to pick them.

When you are done, press That's the cause. You can change the wording first. The problem moves to Fixing. Press Save for later if you want to come back to it.

Look at the process, not the person. "Nobody checks the pack" is something you can fix. "Sam was careless" is not.

Press Help me with this in the step for tips. When AI help is on, FixLoop can suggest answers or causes. You pick, or write your own.
