Report a problem from your phone or computer
Write it or say it in your own words, add a photo, then check the write-up.
FixLoop runs in the web browser, so it works on any phone, tablet or computer. There is no app to install.
- Press Report a problem. On a phone, it is the + button at the bottom.
- Write what happened the way you would say it. Messy is fine. If your browser supports it, you can tap Say it and speak instead.
- Tap Add photo to add photos or a PDF. Each file can be up to 8 MB.
- Fill in Where and When. Tap a place you have used before so the list stays tidy.
- Pick How serious: Minor (small slip, no harm), Major (could have hurt someone or cost money) or Critical (someone was hurt or at real risk).
- Press Next: check the write-up. FixLoop splits your words into a title, What happened, Make safe / Containment and Supporting details.
- Change anything that is wrong, then press Looks right, save report.
When AI help is on, FixLoop writes the draft for you and may ask a few quick questions. They are optional. Skip them if you are busy.
The owner gets an email about the new problem, unless they reported it. Next, the problem waits at Root cause.
Do not include customer or patient names, or health details. The form shows a reminder that fits your type of business.
